The letter must contain your name, address and fax number and
contact information for the other party; including contact name,
address, phone and fax numbers. Please be sure to include the type of
agreement and whether it is a negotiations or grievance case. You
should also notify the other party that forms have been requested.

You can download it to your computer (PDF version), print it, fill it in manually and fax it to us.
